3.所在国家（地区）主管当局对其推荐企业的检疫、卫生控制实际情况的评估答卷；
4.所在国家（地区）主管当局对其推荐的企业符合中国法律、法规要求的声明；
5.企业注册申请书，必要时提供厂区、车间、冷库的平面图，工艺流程图等。

四、审批
1.国家认监委应当按照工作程序对评审报告进行审查，做出是否注册的决定。符合注册要求的，予以注册，并书面通告境外食品生产企业所在国家（地区）的主管当局；不予注册的，应当书面通告境外食品生产企业所在国家（地区）的主管当局，并说明理由。
2.国家认监委应当定期统一公布获得注册的境外食品生产企业名单，并报国家质检总局。
3.注册有效期为4年。

二、需提供的材料
1.出口商或者代理商应当通过备案管理系统填写并提交备案申请表；提供出口商或者代理商名称、所在国家或者地区、地址、联系人姓名、电话、经营食品种类、填表人姓名、电话等信息，并承诺所提供信息真实有效。

三、信息变更
